UNIT: Position Summary
Serves as a clinical and administrative liaison between NYP Network Hospitals and NYP Cornell or NYP Columbia physicians. Works collaboratively with all members of the team of the assigned center and units (Cardiac, Neuro, and Medicine) to ensure high level patient care, optimal physician and consumer satisfaction and promote NYPH Medical Service Line to existing and potential network hospitals. Responsible for coordinating all patient transfers for assigned areas as well as facilitate hospital based patient transfers to and from New York Presbyterian Hospital with the Access Transfer Center.
The Access Clinical Coordinator (Transfer Center RN) will facilitate the transfer of patients from other facilities. The Access Clinical Coordinator (Transfer Center RN) must triage calls, connect attending physicians to conference regarding case, secure a bed, facilitate RN to RN report, activate EMS. The Access Clinical Coordinator (Transfer Center RN) must be critical care trained. The candidate must have excellent communication skills.
