Required Qualifications

Position: Patient RN Navigator - Orthopedic Program This position will not be providing direct nursing care. Day shift, 8 hour shift 0730-1600 BLS, ACLS, PALS required Centralized orientation in Petaluma!! Please review AMIE oreintation closely!!!! The Float pool will float between St. Joseph Hospital of Eureka and sister facility Redwood Memorial Hospital The Patient Navigator will conduct clinical evaluation of patients in accordance with professional licensure. Evaluations include but are not limited to pre and post-operative care of total joint patients. The primary responsibility of the Total Joint Patient Navigator is to guide and coordinate patients through the entire continuum: preoperative, surgical and postoperative care. The Patient Navigator will collaborate with the medical staff to provide individualized care to the patient and perform all duties involved in the management of the daily operations of the joint replacement program. The Patient Navigator will be responsible for collecting program outcome data and evaluating the effectiveness of the program, and report out to clinical based committees. A strong focus is placed on patient and community education/outreach, as well as hospitality and patient satisfaction.
